The only known copies are imperfect. The New York Public Library copy was recovered by Wilberforce Eames from the Bradford binding of another work and is fragmentary. Title page transcription completed by Mr. Eames. A second copy, held by Michael Zinman, lacks the title page but is otherwise complete.

Error in paging: page numbers 39 and 40 repeated.
