520 $aThis report documents the final portion of investigations recommended by the Board of Inquiry convened to determine the cause of the piping failure at Anita Dam. Two previous reports by this author have presented the results of other portions of the extended investigative program. Drilling, sampling, and in situ testing for this phase of subsurface explorations were completed during May of 1997, utilizing personnel and equipment of the Bureau of Reclamation and the Bureau of Land Management. Laboratory testing was performed by two commercial laboratories in Billings, Montana. Investigations consisted of drilling a total of 5 holes in the embankment and foundation of the dam to determine characteristics of both the embankment and foundation materials. Four of the holes were drilled along the dam axis and the other was completed near the vertical risers for the valve control structure and the overflow spillway. Standard Penetration Tests (SPT) were conducted in the embankment and foundation to detect soft zones and provide representative material for moisture determinations, gradations and Atterberg Limits. Two undisturbed samples were collected for one-dimensional consolidation testing near the base of the vertical risers, and an additional sample of embankment was tested for the presence of dispersive clays. Double porous tube piezometer installations were completed in two of the drill holes. .--Page 1.
