The dedication is signed: Walter Charleton.
The two plates are a portrait of Charleton and an engraving of the "Theatrum Cutlerianum". The engraved portrait is signed: D. Loggan ad vivum delin. et. sculp: 1697.
Although the work was intended to include six praelections, only the following were published: I. Of nutrition; IV. Of life; V. Of fevers; VI. Of motion voluntary.
The first leaf bears an order to print on verso, dated March 25, 1679 and signed John Micklethwait, President.
"Advertisements" on p. 12 are errata notes.
