"Done at Louisville, in Convention, the sixteenth day of May, in the year of our Lord one thousand seven hundred and ninety-five ..."--p. 4.

The only known copy, held by the De Renne Library, is bound with The Constitution of the state of Georgia. Augusta : John E. Smith, 1789 (Evans 21850) and is endorsed in ms: Georgia. Secretary's office. 21 Jan. 1797. I do hereby certify the within from so 1. to 7. to be a true copy of the amendments to the constitution of the state of Georgia entered into on the 16th May 1795. Jno. Milton, secy.

Ascribed to the press of John E. Smith by Evans, who suggests 1796 as date of publication. Smith advertised "The constitution of the state of Georgia, with the additions and amendments" as "Published and for sale at this office" in the Augusta chronicle, Nov. 12, 1796. Smith may have been advertising the present item bound with the 1789 edition of the constitution.
